Veterans Honor Program

Park Terrace has a Veterans Honor Program serving those who have served.  We instill a trained interdisciplinary team approach to Veteran Care.   Park Terrace offers a variety of Enrichment programs and information for care givers.

Admissions Process

Our dedicated team will work with the resident and family upon admission to assure that a resident is given the best care from the very first moment of arrival. This includes a complete evaluation of the resident’s medical history, an assessment of therapeutic needs, as well as an analysis of lifestyle such as nutrition, exercise, and social needs. Once admitted to Park Terrace Nursing and Rehabilitation, the entire team of clinicians will assess, evaluate and collaborate, to design a carefully crafted individualized plan of care with specific objectives and goals outlined, to ensure that every aspect of a resident’s needs are addressed and met. Throughout, Social Service will maintain close contact with the resident and family to continuously work toward Park Terrace’s goal of providing excellent care and patient satisfaction, throughout your journey along the road to recovery.

To arrange for admission to Park Terrace:

From the Hospital – Inform the hospital social worker or case manager that you have chosen Park Terrace for your post-hospital care. They will send medical information and communicate with the Park Terrace Admissions staff to coordinate a transfer. Also, call the Park Terrace Admissions office at 419.531.4465 to notify them of the pending admission. The admissions team will collaborate with you, the hospital and your attending physician to coordinate transfer to Park Terrace.

From Home –  just call the Park Terrace Admissions office at 419.531.4465 they will guide you through the process and coordinate transfer to Park Terrace.

From an Acute Rehab, L-TAC or other Nursing Facility – notify the social worker or case manager that you wish to transfer to Park Terrace. They will contact the Park Terrace Admissions office and work closely with the other facility to facilitate transfer to Park Terrace.
