Located in Toledo, Ohio near Maumee Bay State Park and the shores of Lake Erie, Park Terrace Rehabilitation Center offers comprehensive rehabilitation, therapy, skilled nursing, and long-term care under one roof. Specialty programs include our state-of-the art Pulmonary Care Unit, our Dialysis Program, our Memory Care Program, and our Family Care Program that focuses on whole-family wellbeing.
